CITY OF MONASH INDEMNITY DECLARATION

I agree to allow the participant ('the participant') to join in Monash Youth Services Youth Programs for the year of 2024.

I agree to release, indemnify and keep indemnified the City of Monash, its employees and/or volunteers, its officers and servants (‘the Council’) from and against all actions, costs, claims whatsoever which may be brought or made or claimed against the Council, in connection with or arising out of or in relation to the Participant’s participation in the above mentioned program. This release and indemnity shall be reduced proportionally to the extent that any act or omission of the Council directly contributed to the loss or liability.

I authorise the Council to administer or cause to be administered such first aid, medical and like treatment that they consider is needed for the welfare of the Participant, provided that such person is legally able to administer such treatment. I further agree to hold the Council harmless for such medical treatment provided to or sought for the Participant and I will be totally responsible for any costs of such treatment.

I declare that the Participant is in good health and agree to advise immediately in the event of the Participant contracting any ailment or medical condition that is likely to be detrimental to or impact on their ability to take part in the above mentioned program and/or the health of other participants.

I acknowledge that the Participant is responsible for all personal belongings that they bring with them to the above mentioned program. I agree that the Council will not be responsible for any personal items of the Participant, including but not limited to any damage, loss or theft.

Monash City Council (“Council”) collects personal information via this form to provide you with the service you are seeking and/or to fulfill its statutory responsibilities, and for related purposes which you may reasonably expect.
